1. Your Privacy Rights
This notice supplements our Privacy Policy and describes the rights of residents of California and other U.S. states with comprehensive privacy laws regarding personal information handled by ReimburseRPM LLC d/b/a SimplyRPM (“SimplyRPM”). Note that information governed by HIPAA or other sector-specific laws may be exempt from some of these rights; see our HIPAA Notice.
2. Categories of Information We Collect
In the past 12 months, we may have collected the following categories of personal information: identifiers (such as name, email, and account identifiers); customer records (such as contact details); health and medical information you provide through the Service; internet and device activity (such as usage and log data); and inferences drawn to operate the Service. We describe the sources, purposes, and disclosures for these categories in our Privacy Policy.
3. We Do Not Sell or Share for Advertising
We do not sell your personal information, and we do not share it for cross-context behavioral advertising, as those terms are defined under California law. We have not done so in the preceding 12 months, including with respect to information about individuals under 16.
4. Your Rights
Subject to applicable law and verification, you may have the right to:
- Know / access the personal information we have collected about you and how we use and disclose it;
- Correct inaccurate personal information;
- Delete personal information we have collected, subject to exceptions;
- Portability, obtain a copy of certain information in a portable format;
- Opt out of any “sale” or “sharing” (although we do not sell or share); and
- Non-discrimination, not be discriminated against for exercising your rights.
5. How to Exercise Your Rights
To make a request, email us at [email protected] with the details of your request. We will take steps to verify your identity before responding, and we will respond within the timeframes required by law. If you use the Service through a health care provider, we may direct your request to that provider or fulfill it on their behalf as required by our agreements.
6. Authorized Agents
You may use an authorized agent to submit a request on your behalf. We may require the agent to provide proof of authorization and may still ask you to verify your identity directly.
7. Other U.S. State Rights
Residents of states such as Virginia, Colorado, Connecticut, Utah, and others with comprehensive privacy laws may have similar rights, including to access, correct, delete, and obtain a copy of personal data, and to appeal a decision on a request.
