2026 Medicare RPM CPT codes at a glance
- CPT 99453 – Initial device setup and patient education. Billed once per episode of care (~$22).
- CPT 99454 – Device supply with daily recordings or programmed alerts, 16-30 days of readings in a 30-day period (~$47).
- CPT 99445 – New for 2026. Device supply for 2-15 days of readings in a 30-day period (~$47). Patients who miss the 16-day threshold are no longer unbillable.
- CPT 99457 – Treatment management, first 20 minutes per month with at least one interactive communication (~$52).
- CPT 99458 – Each additional 20 minutes of treatment management (~$41).
- CPT 99470 – New for 2026. Treatment management, first 10 minutes per month with interactive communication (~$26). Shorter clinical months are now billable too.
