SMART on FHIR EHR Integration for Remote Patient Monitoring

SMART on FHIR is the open standard that lets third-party health apps run securely inside an electronic health record. SimplyRPM was built on it from day one, and it changes what remote patient monitoring feels like for a clinical team.

What SMART on FHIR means for your RPM program

  • One login, zero portals. Clinicians launch SimplyRPM from inside the patient chart. Patient identity and context come along automatically – no duplicate lookups, no copy-paste.
  • Vitals where they belong. Device readings flow into the record as FHIR Observations, so blood pressure trends live next to labs and meds instead of in a silo.
  • Security by standard. Authorization uses OAuth 2.0 scopes defined by the SMART framework, so your organization controls exactly what the app can read and write.
  • Works across FHIR-enabled EHRs. Because SMART on FHIR is required of certified EHRs under the 21st Century Cures Act API provisions, the same integration pattern reaches Epic, athenahealth, eClinicalWorks, and other major systems.

How the integration works

  1. Your EHR admin registers SimplyRPM as a SMART app (we provide the launch URL and scopes).
  2. A clinician opens a patient chart and launches SimplyRPM from the app menu.
  3. SimplyRPM receives the patient context via FHIR, shows device readings, alerts, and time logs.
  4. Summaries and billing documentation write back to the chart.
